AcknowledgmentRegistration fees paid to The Zones are non-transferable and non-refundable, including my child withdrawing from the selected class. I understand I will not be issed a refund or credit for registraion fees paid to The Zones.

Media ReleaseThe Zones takes pictures and video periodically for social media, promotional and advertising purposes. By checking this box, you agree to being photographed or have video taken and used for these purposes in the form of print or digital copies. No compensation or royalties will be provided as a result of photo and media taken. I may opt out of this Media Release with written notice to The Zones.

Medical ReleaseIn the event of an accident, injury or illness, if a parent or guardian cannot be reached, I hereby give my permission for a healthcare provider to make resonable medical decisions deemed necessary by a licensed physician and to transfer to any hospitals reasonably accessible. I understand and agree that The Zones does not assume responsibility for any costs which arise out of or in connectioin with such an authorized emergency medical treatment.

Policy AgreementBy enrolling my child in classes or programs at The Zones, I acknowledge that I have read, understand, and agree that my child will adhere to all policies, procedures, and behavioral expectations established by The Zones while participating in any class, activity, or program. I understand that The Zones’ full Parent Handbook, including behavior policies and program guidelines, is available on The Zones’ website and that it is my responsibility to review and discuss these expectations with my child.
